Dell Medical School Registrar Requests

Please complete the required form(s) for items needed from the Office of the Registrar. Leaving requested information blank or not providing proper form(s) for processing will result in delays.

There are no unofficial transcripts, academic summaries, or digital requests of a DMS transcript. However, an enrollment certification can be processed and scanned to UT Box for student retrieval.

Enrollment Certification

An enrollment certification is an official document that confirms attendance, awarded degrees, current and past enrollment, expected graduation date, and other parts of a student academic record. 

You may need certification of enrollment to defer student loans, provide proof of enrollment for health insurance, verify degrees for prospective employers or show proof of academic standing to enroll in another institution.

Official certifications are printed on security paper with the university seal and the registrar's signature.

 

Transcript Request Form

Complete the Transcript Request Form (to open link in a new tab, right click on link and select "open link in a new tab") and submit below, handwritten signatures please!!

Dell Medical School transcripts include the following:

  • Dell Medical School coursework
  • Dual degree coursework taken as a medical student
  • Degrees awarded

If you would like a transcript of other undergraduate, graduate, extension, or correspondence courses completed at UT Austin, please indicate on the request form.

All transcripts (including Dell Medical School) are $20 each.

Transcripts are mailed when payment is processed.

 

Form Provided Certification

This request is a certification and/or form provided by you the student and/or alumni for completion requiring dean's signature and/or registrar's signature (e.g., State License Forms, Medical Education Verification, Intern/Resident Forms, etc.).

Please read your form carefully, Form D requires an official transcript that is another request, form can be found above. If your certification and/or form requires photo of student, this MUST be provided by you.

Complete the Graduation Processing Form (to open link in a new tab, right click on link and select "open link in a new tab") and attach BOTH your form and the Graduation Processing Form when submitting below. The Graduation Processing Form must be completed to release information to you or a 3rd party and attached WITH the certification and/or form you need our office to complete. 

Diploma Photocopy

Complete the Graduation Processing Form (to open link in a new tab, right click on link and select "open link in a new tab") and submit below to receive a certified copy of your diploma needed for certification, verification, licensure, etc. Handwritten signatures please!

If you need your diploma photocopy notarized please make sure to indicate that on the form.
