To request an academic summary, you can mail or fax a letter to the Office of the Registrar including the following information:
- Date of request
- Full name under which you were enrolled
- Current name, if different from above
- EID
- Date of birth
- Dates of attendance
- Current mailing address
- Daytime phone
- Signature in both names if different
