Who to call to change the name on a diploma

To order a replacement, fax our order form or a letter to 512 475-7515, or mail our form or your letter to the University of Texas at Austin, Office of the Registrar, attn Diploma Services, PO Box 7216, Austin TX 78713-7216. If mailing or faxing a letter instead of our order form, the letter must include

  • the full name of the student as recorded when last enrolled at UT Austin;
  • the student's birthdate;
  • all degrees received by the student;
  • if multiple degrees were received, which diplomas are being ordered;
  • the student's graduation year;
  • the student's UT EID electronic identifier;
  • the mailing address where the replacement diploma is to be sent;
  • an email address and phone number to call for any questions we may have about the order; and
  • the signature of the student who received the degree for the diploma being replaced.

The fee for a duplicate diploma is $10. Payment can be made by check, money order, Master Card, Visa or Discover, made payable to the University of Texas; if paying by credit card, include the card's number and expiration date.

A financial bar on a student's record will prevent processing a diploma order for that student. (Go to the What I Owe website to check for a financial bar or to pay financial obligations online. An EID and password are required to access student accounts.)

It takes about a week for us to print, package and ship a diploma once an order is received. We ship it by first class mail.

For diplomas originally issued before 2000, the replacement diploma will bear the signatures of current officials, with the notation Reissued appearing below the date awarded.

Name changes

All name changes submitted with diploma requests must be accompanied by supporting documentation. If the change of name is due to marriage, a certified copy of the marriage license is required. Certified copies of court-ordered name changes are acceptable. Legal name change requests due to divorce must be sent with a certified copy of the divorce decree which reinstates the maiden name. Signed name change requests will be processed and documentation returned to the sender. Name changes will be reflected on the student’s permanent record and on the reissued diploma.

Special requests

We can notarize your diploma at no additional charge. Those graduates requesting diplomas for employment purposes overseas may need to investigate what authentication may be required by the country where the diploma will be presented. Diplomas may be sent overnight at your expense; our office uses Federal Express, and those fees will be billed to your credit card.

Should any country requiring a diploma also require an apostille, we can forward your notarized diploma to the US secretary of state’s office. Fees for the apostille will be charged to your credit card.
